Our senior health services are provided by experts in the field of geriatric medicine and we offer programs and resources tailored to meet the needs of seniors in our community.

Health care for seniors

Trinity Health Mid-Atlantic LIFE programs provide all-inclusive medical and social services so seniors can continue living independently at home and in the LIFE day centers. Services are provided at no cost to seniors who qualify.

Home health provides the care you need in the place you love. Our team of registered nurses and licensed rehabilitation therapists will be by your side as you recover or learn to manage your medical condition from the comfort of home.

St. Mary Medical Center’s MG3 Unit is designed to provide comfort and safety for seniors. As a NICHE (Nurses Improving Care for Healthsystem Elders) facility, our specially trained Geriatric Resource Nurses are dedicated to improving health outcomes through a collaborative approach with care management, social work, physical and occupational therapy, the pharmacy and community based providers like LIFE. This facility was designed specifically with seniors in mind and has spacious, private rooms and relaxing colors and décor that feel like home.

Select Specialty Hospital provides long-term care services at Saint Francis Hospital in Wilmington, Delaware. This facility is designed to treat patients with extended care needs. Seniors in need of long-term hospital care often have conditions like heart failure, renal failure, pulmonary disease and non-healing wounds.

Saint Francis Hospital has received accreditation as a Senior-Friendly Emergency Department from The American College of Emergency Physicians (ACEP). Saint Francis is the first hospital in Delaware to receive this certification.

This certification reflects Saint Francis Hospital’s ability to provide the highest quality medical care for older adult patients. It recognizes the hospital’s commitment to providing clinically excellent care, ensuring optimal transitions from the Emergency Department to other settings of care (inpatient, rehabilitation, home care, long-term care), and supporting geriatric-focused quality improvement.

To be accredited, hospitals must be able to provide a set of key services and accommodations for the older adult population, including:

  • Geriatric-specific, patient-focused continuing education for all staff
  • Additional equipment and renovations focused on the needs of elderly patients
  • Case management and social work presence in the ED to assist with care and discharge planning.
