Saint Francis Palliative Care Services
Palliative care services are available for inpatients and outpatients and can include:
- Goals of care discussion
- Advance care planning
- Education, counsel and support
- Symptom management
- End-of-life and hospice referral as appropriate
A nurse certified in palliative care and hospice provides consultation services for palliative care needs. We take a unified approach to palliative care. All staff within Saint Francis, including medical, nursing, social work and pastoral care, support our program. In addition to the support of other resources, we also utilize standardized training resources from the Center to Advance Palliative Care (capc.org) for medical and staff training.
To provide the most robust care for our patients at Saint Francis:
- We collaborate with local and regional care providers for palliative care, as well as specialty services related to mental health and substance use disorder clinicians
- Advance care planning is a shared opportunity at Saint Francis between our pastoral staff, palliative care and care management teams
- All medical and nursing staff contribute palliative care support
