Newtown Square, Pa. – (September 5, 2023) – In calendar year 2022, Trinity Health Integrated Care (THIC) Accountable Care Organization (ACO), a Medicare Shared Savings Program (MSSP) based in Livonia, Mich., coordinated high quality care for over 130,000 lives, saving Medicare $56M, with earned shared savings of $41.2M to THIC ACO and its participating providers.

Trinity Health Mid-Atlantic Clinically Integrated Network (CIN), which operates within THIC ACO, includes three local member organizations: Quality Health Alliance (QHA), Mercy Accountable Care (MAC) and Delaware Care Collaboration (DCC). QHA, MAC and DCC, whose partners include Trinity Health Mid-Atlantic hospitals, Trinity Health Mid-Atlantic Medical Group physicians and a number of other independent affiliated community-based primary care physician practices, subsequently earned $8.14M in shared savings from CMS as a result of this strong performance, which was largely driven by more effectively managing:

  • Avoidable hospital stays;
  • Avoidable ambulatory-sensitive visits to the Emergency Department; and
  • The length of stay for patients admitted to skilled nursing facilities.

Over 90 physician practices from Trinity Health Mid-Atlantic participated in the MSSP during the 2022 calendar year, providing care to more than 26,800 beneficiaries. Trinity Health Mid-Atlantic CIN also maintains a robust care coordination team of nurses, social workers, behavioral health specialists, health coaches, and other health care providers dedicated to providing comprehensive medical and non-medical management to its beneficiaries.

Trinity Health Mid-Atlantic CIN continues to emphasize the importance of preventative screening, active chronic disease management and the delivery of the highest quality of care to slow or prevent disease progression and the need for expensive hospital-based services.
