Northeast Philadelphia — (January 3, 2020) — The Transitional Care Unit (TCU) at Nazareth Hospital was ranked as a high-performing nursing home in Pennsylvania by U.S. News & World Report in its recently revealed 2019-2020 rankings.
The TCU at Nazareth Hospital received an overall rating of 5 out of 5. The 28-bed TCU facility is not part of a continuing care retirement community. Rather, it provides care that helps patients prepare to make the transition from an acute/inpatient hospital stay to home — safely and with appropriate follow up.
“The Nazareth Hospital Transitional Care Unit team is proud to be acknowledged for its dedication to high-quality, patient-centered care,” said Dr. Genevieve Skalak, DO, FACOI, Physician Advisor and Medical Director for the Transitional Care Unit at Nazareth Hospital. “We strive to provide excellent, compassionate care for our community, and this recognition is a testament to our success in serving our patients and colleagues.”
Just under 3,000 nursing homes in the country earned the designation, which evaluates a nursing home’s quality of post-acute care for patients recovering from a hospital stay, such as after a stroke, heart attack, infection or accidental injury, and patients receiving post-operative care.
