LIFE Programs
Living Independence for the Elderly
LIFE programs give seniors the support they need to live in their own home or with family. Each participant receives full circle medical and social care based on an individual care plan. LIFE services are free to seniors who qualify. Visit the sites below to learn more about the LIFE program in your community. These LIFE programs are a part of Trinity Health PACE.
