Delaware Care Collaboration, a Partnership of Saint Francis Hospital and the Medical Society of Delaware, will receive the shared savings for Quality and Care Coordination Performance in 2018
Wilmington, Del. – (October 9, 2019) – Delaware Care Collaboration (DCC) achieved $10.1M million dollars in savings for the Centers for Medicaid and Medicare Services (CMS) in 2018 – a 10.85% savings rate. DCC achieved the 12th highest savings rate percentage among 548 Medicare Shared Savings Program (MSSP) ACOs. The organization will receive nearly $4.9M in shared savings as a result. DCC earned a 98.64% overall quality score under MSSP pay-for-performance metrics.
Delaware Care Collaboration participates in the Track 1 MSSP, an alternative payment model aimed at increasing quality and affordability of healthcare. CMS shares savings with providers who succeed in meeting quality, performance and cost efficiency metrics for an assigned Medicare fee-for-service beneficiary population. DCC is the only ACO based in Delaware to have achieved savings.
“Delaware Care Collaboration’s outstanding performance is the direct result of the engagement, collaboration and dedication of our participating providers, working in concert with clinical and care coordination professionals to ensure the Medicare beneficiaries we care for receive the right care at the right time in the right place,” said Dan Sinnott, President and CEO of Saint Francis Hospital.
“The Delaware Care Collaboration (DCC) started with the goal of improving the quality of healthcare for our patients. The DCC team works to wrap around the necessary services to improve the patient experience and decrease unnecessary health care costs. Our practice liaisons work directly with our practices to advance practice transformation efforts to improve quality. The care management team focuses on the highest risk and most vulnerable patients to provide education, communication and complex disease specific case management while breaking down barriers related to the social determinants of health. The data analytics focuses our efforts to where we can make the most impact. The team approach to care delivery and the dedication of the practices and their staff to the mission of the DCC was critical to our success,” said Delaware Care Collaboration Medical Director Robert Monteleone, MD.
Over 10 million beneficiaries were cared for in 2018 by 548 Shared Savings Program ACOs. The National Association of ACOs reports that according to CMS data for that year, “66 percent of ACOs saved Medicare money, compared to CMS-set spending targets or benchmarks, and 37 percent saved enough money to earn shared savings bonuses. Both of these numbers are increases from previous years. ACOs collectively saved Medicare $1.7 billion last year alone, and $739 million after accounting for shared savings bonuses and collecting shared loss payments.”
