Heart Failure PUMPED Program
The Quality Health Alliance (QHA) PUMPED program is a 90-day, patient-centered initiative designed to help individuals with heart failure improve their quality of life. With a focus on personalized care, education and support, the program meets patients where they are—offering flexible, compassionate guidance every step of the way.
Through weekly check-ins—by phone or in-home, based on your schedule—you’ll connect with a dedicated team of health care professionals who are here to help you feel confident and cared for. They’ll provide you with needed materials, devices and assessments to aid in your journey.

Patient eligibility for referring physicians
- The cohort of patients identified for inclusion are patients attributed to the QHA Clinically Integrated Network’s (CIN) value-based programs
- Diagnosis of heart failure with or without comorbid conditions
- Exclusions: End-stage renal disease, chronic kidney disease, sustained Ejection Fraction (EF) less than 25% (6 months or greater)
